What Is ERP?
ERP, or Exposure and Response Prevention, is the gold standard treatment for OCD. It's one of the most researched and effective therapies available for intrusive thoughts, compulsions, and the anxiety cycle that keeps people stuck.
Here's the simple version. OCD works by convincing you that if you just check one more time, avoid the thing, or get enough reassurance, the anxiety will go away. And it does, temporarily. But the relief doesn't last, and the cycle gets stronger.
The goal isn't to get rid of the thoughts. It's to stop letting them run your life.
ERP works differently. Instead of giving the OCD what it wants, we gradually and carefully practice sitting with uncertainty without doing the compulsion. Over time, your brain learns that the anxiety passes on its own, and the intrusive thoughts lose their grip.
It's not about white-knuckling it. It's about building real tolerance, one step at a time.
Who Is ERP For?
ERP is the first-line treatment for OCD, but it can also be highly effective for a range of anxiety-driven patterns. These include OCD with intrusive thoughts, compulsions, mental rituals, and reassurance seeking.
It's also effective for Pure O, which is OCD that feels entirely internal with no visible compulsions. Health anxiety, social anxiety, phobias, and generalized anxiety that shows up as worry loops that feel impossible to break are also areas where ERP can make a real difference.
If you've tried therapy before and felt like it didn't touch the OCD, there's a good chance ERP wasn't part of the picture. It makes a real difference.

What Does ERP Actually Look Like?
A lot of people come into ERP expecting it to be terrifying. It's not, at least not when it's done right.
We start slow. Before we do any exposure work, I take time to understand your OCD, the specific thoughts, the specific compulsions, and how the cycle plays out for you. Everyone's OCD looks different and treatment should reflect that.
From there, we build what's called an exposure hierarchy. Think of it as a ladder of situations, starting from mildly anxiety-provoking and working up gradually, at a pace that challenges you without overwhelming you.
During exposures, I'm with you. We process what comes up, build skills to tolerate the discomfort, and track your progress so you can see how far you've come.
ERP is active work. It's not just talking about OCD. It's doing something about it.
Why Work With Me
I'm Kevin Hayden, LICSW and founder of Whole Mind Therapy. I have over 20 years of experience working with adults navigating anxiety, OCD, and the patterns that keep people stuck.
I received specialized training in ERP through the International OCD Foundation (IOCDF) and have worked extensively with adults who've struggled with OCD, including those who've seen multiple therapists and never gotten real traction.
My approach is direct and practical. I'm not going to have you talk about your intrusive thoughts indefinitely without a plan. We're going to build skills, do the work, and measure what's changing.

ERP and Other Approaches
ERP is the foundation of OCD treatment in my practice, but it doesn't work in isolation. I also integrate CBT (Cognitive Behavioral Therapy) to identify and shift the thought patterns that fuel OCD, EMDR for clients whose OCD is connected to past experiences or trauma, and mindfulness as a tool to observe thoughts without reacting to them.
The goal isn't just symptom reduction. It's building a life where OCD doesn't run the show.

Frequently Asked Questions
I've heard ERP is really hard. Should I be worried?
It's challenging and I won't sugarcoat that. ERP asks you to face what feels uncomfortable on purpose, which goes against every instinct OCD has trained into you. But we go at a pace that's manageable and I'm with you through it. Most people find that the anticipation of ERP is scarier than the work itself.
I'm not sure if I have OCD. Can ERP still help me?
ERP was developed for OCD but has strong evidence for other anxiety-driven patterns too, including health anxiety, phobias, and social anxiety. If you're caught in a cycle of anxiety leading to compulsions or avoidance, followed by temporary relief and then more anxiety, ERP is likely relevant for you. Do you offer online ERP therapy in Massachusetts?
Yes. All sessions at Whole Mind Therapy and Counseling are conducted online, serving adults throughout Massachusetts. Online ERP is just as effective as in-person. The work is the same, just done from wherever you are.
How is ERP different from regular talk therapy?
Talk therapy helps you understand your patterns. ERP helps you change them through direct practice. For OCD specifically, insight alone rarely moves the needle. You need to actually practice responding differently. That's what ERP is designed to do.
